In order to process payment for our reliable subcontractors in a timely fashion, please support our efforts by submitting the following documentation prior to providing your services:

• A completed W-9

• A certificate of general liability naming Struble Construction, Inc (1630 N Blackwelder Ave #6, Oklahoma City, OK, 73106) as the certificate holder

• A certificate of workers’ compensation insurance OR a workers’ compensation affidavit filed with the Oklahoma Workers’ Compensation Commission

• A completed Contractor Information Form
